Return / Refund Policy

At EVOLV Plastic Surgery & Medical Aesthetics we work with each patient to create individualized programs and review likely outcomes, benefits, and risks associated with each treatment. We offer state-of-the-art services and products as well as value-added treatment package options so each patient may choose the approach that best suits their goals and budget.

Because of the additional staff time and costs involved, refunds cannot be given once services and/or gift cards are purchased.  However, to ensure our patients always receive the best experience at EVOLV Plastic Surgery & Medical Aesthetics unused service values (cash equivalent for the remaining amount of a treatment package lesser the discounts) can be applied to other full-priced treatments and/or products here at EVOLV.  For these same reasons, all injectable treatment sales (such as, but not limited to Botox, dermal fillers, laser treatments, facials, body service treatments, or chemical peels) are final; refunds or credits cannot be offered once treatment is completed.

For retail products, all sales are final.  Should you have an adverse skin reaction to one of the products or a product malfunction, we will provide a credit to your account.  If an adverse reaction is the reason for the return, please call our office at 419-720-2008. We will have your provider contact you directly to help complete the form that must be completed for us to submit to the brand of the product.  All returns will be for merchandise credit or exchange only.  Please note, we are unable to accept returns on all prescription items such as tretinoin and Latisse, as well as cosmetics from Jane Iredale and Colorescience.

Medical aesthetics is a highly specialized and subjective profession and how you may respond to a given treatment will vary from person to person.  It is virtually impossible to predict results and therefore payments made for services are for treatments to be performed — not for a specific result.  Our EVOLV professional staff is highly trained and experienced, and we always strive to achieve the absolute best results and value that we can for you.
