When scheduling a consultation, we do require a $150 non-refundable deposit for appointments with Dr. Barone to reserve your appointment time. This is most efficiently paid by credit card, but we will work with you if you prefer another payment method. This deposit ensures that valuable consultation time is available to those who are truly committed to learning about their aesthetic options and will be applied to your choice of surgical procedure or aesthetic service with Dr. Barone. To efficiently accommodate all our patient’s needs, we request at least 48-hours’ notice in the event you need to cancel or reschedule an appointment. This gives us adequate time to reschedule patients appropriately and avoid disruption in our patient care.

What to Expect During Your Visit
We believe that patient education is a very important part of the decision-making process, and our consultation visits tend to be very informative and comprehensive. Please expect to spend 1-2 hours with us for your initial consultation. During your appointment, you will meet our Patient Coordinator, Sandy who will introduce you to the practice and will also discuss your goals. You will then meet with one of our nurses who will review your health history information and then begin discussing possible procedure options with you. Dr. Barone will then meet with you and discuss specific recommendations to meet your goals after his examination. We will take photos at your consultation visit.
Facial Rejuvenation Considerations
If you are consulting with Dr. Barone for facial concerns, please consider that we may need to remove your makeup if we are taking facial photos or Vectra Imaging. We carry the mineral cosmetic brands Jane Iredale and Colorescience if you wish to reapply for makeup after your visit. One of our makeup artists would be happy to select the best products for your skin. We may also spend a considerable amount of time discussing your skincare, as we know that quality skincare is essential for skin health and will provide you with an optimal post-procedure result and help you to maintain those results for years to come.
Rescheduling & Cancellation Policy
At EVOLV Plastic Surgery & Medical Aesthetics, our goal is to provide you with quality care. To do so, appointments are offered in a timely fashion and the appropriate amount of time is scheduled for you to discuss your concerns with one of our providers. A missed appointment not only delays your care but also prevents us from offering that appointment to another patient who could have been seen at that time.
We do understand that unforeseen circumstances may arise, and you may need to reschedule your appointment. We kindly request you notify our office within 48 hours of your appointment. If an appointment is missed or is not canceled within 48 hours of the scheduled appointment, a $50.00 fee will be assessed.
We accept returns within 14 days of the original purchase due to an adverse reaction as well as a product malfunction. If an adverse reaction is the reason for the return, please call our office at 419-720-2008. We will have your provider contact you directly to help complete the form that must be completed for us to submit to the brand of the product.
All returns will be for merchandise credit or exchange only.
Please note, we are unable to accept returns on all prescription items such as tretinoin and Latisse, as well as cosmetics from Jane Iredale and Colorescience.
When we reserve time for you, we require all that time to provide you with the best quality of care possible. Even one late patient can cause the entire daily schedule to fall behind. If you arrive more than ten minutes late, we reserve the right to shorten or reschedule the appointment. Our providers strive to stay on time, but we realize we also run late. Sometimes a patient comes in that requires additional attention, which can cause delays. We will do our very best to notify you when this occurs.
