Rescheduling & Cancellation Policy

Our patients and their care are at the heart of our practice at EVOLV. To ensure everyone has fair and efficient access to the care they need, we have established the following cancellation policy. *Excludes New Patient Consults for Dr. Barone.

Appointments are reserved specifically for you, and when one is booked, it represents valuable time that could have been offered to another patient. As a courtesy to our providers and other patients, we kindly ask that you provide at least 24 hours’ notice if you need to cancel or reschedule your appointment.

Effective immediately, we will require a card on file to secure your appointment.

If an appointment is canceled or rescheduled within 24 hours of the scheduled time, or if you do not show up, a $50 cancellation fee will be charged to the card on file.

Additionally, if you arrive more than 10 minutes late, we may need to reschedule your appointment to prevent disruptions to other clients.
